During endodontic treatment, dental root canal and the bone around the top of the roots treatment is performed. When the tooth crown or its’ root because of deep cavity decay, large fillings or injury appear violations, bacteria and their toxins reaches the pulp tissue, which contains blood vessels, nerve tissue of the tooth, and lymphatic vessels, and it becomes inflammatory. Acute spasmodic pain occurs due to inflammation. Sometimes in such cases, there are no clear symptoms.
In order to eliminate the pain and prevent the spread of bacteria and toxins through the canals towards the bone, endodontic treatment is required.
When tooth pulp becomes co-infected and irreversibly damaged, the bacteria begin to multiply and spread along the nerve towards the tip of the root. Bacterial toxins, passing through a hole to the top of the root, usually cause infectious lesions in the deeper tissues, i.e. the abscess – purulent inflammation, which occurs at the very apex of the root.
Symptoms, which show that in fact endodontic treatment is necessary, is a strong pain, especially while chewing or biting, swollen gums, long -lasting tooth pain when the tooth is irritated with cold or hot stimulus. If you notice that your tooth became darker or has changed the color, the gums around the damaged tooth got thicker, red or swollen, also the ache at night, you should get worried. Of course, it happens that the symptoms are barely noticeable even if there are severe endodontic diseases.
Primary and secondary endodontic treatment is carried out. During the initial endodontic treatment tooth root canals are sealed and cleaned chemo mechanically. This procedure takes quite a lot of patience and accuracy. When the root canals are sealed, it is necessary to prepare for further treatment, i.e. the crown of the tooth is restored with a filling material, or with a post and core. Later, if the filling contains about two -thirds of the crown, prosthodontics is applied. During the secondary endodontic treatment, i.e. tooth root canal re-treatment, the canal is re-cleaned and the old root canal fillings are removed because of the previous unsuccessful canal treatment. Then the root is re-filled. If the canal is badly cured and permeable, bacterium and toxins, that cause violation and chronic pain of the bone around the top of the tooth root, get into alveolar bone. Thus, the purpose of the secondary endodontic treatment is to cure the primary alveolar bone tissue from inflammation and save the tooth. This treatment is particularly complicated and its success is not always long-term. After the treatment, the surveillance by the dental radiological survey is recommended because of the progression of the infection.
If root canal is not cured on time, the tooth can be irreversibly damaged and it has to be extracted. In this case, in order to restore the lost tooth and get a full recovery of chewing and beauty, implantation or prosthodontics is performed.
